What causes melasma?

Melasma is a chronic skin condition that causes brown, gray-brown, or blue-gray patches — typically on the cheeks, upper lip, forehead, chin, and along the jawline. It affects an estimated 5 million Americans, and over 90% of those affected are women, often during or after pregnancy ("the mask of pregnancy"), with hormonal birth control, or simply with prolonged sun and heat exposure.

Unlike sun spots or freckles, melasma involves overactive pigment-producing cells (melanocytes) deep in the skin. The pigmentation can sit at multiple depths — epidermal, dermal, or mixed — which is why generic spot treatments rarely work. Melasma is also notoriously stubborn: it requires specialized protocols, sun protection, and an ongoing maintenance plan to keep it under control long-term.

At Nova, we approach melasma the way it deserves to be approached — as a complex pigment disorder that requires layered, professional intervention. Generic at-home remedies usually fail. Our protocols combine in-clinic depigmentation peels, prescription-grade topicals, controlled microneedling, and rigorous SPF discipline to give you real, lasting clarity.

A multi-layered strategy.

Phase 1 · Depigmentation

Cosmelan Peel

Considered the gold standard for melasma. A clinically-applied depigmentation mask suppresses tyrosinase — the enzyme responsible for melanin production — at the source. Followed by a take-home phase to maintain pigment suppression.

Phase 2 · Stabilization

Tranexamic Acid

Applied topically or microneedled into the skin, tranexamic acid blocks the inflammatory signals that trigger melasma flares — making it one of the most evidence-backed melasma treatments available today.

Phase 3 · Renewal

Microneedling

Gentle controlled microneedling enhances penetration of brightening serums and stimulates healthy cellular turnover — without aggravating pigment-producing cells the way harsh lasers can.

Phase 4 · Maintenance

Topical Protocol

A personalized take-home regimen — typically including a medical-grade tinted SPF 50+, antioxidants, and prescription brighteners — to keep your results and prevent rebound pigmentation.

Why melasma demands a specialist.

Most pigmentation treatments make melasma worse. IPL, aggressive lasers, and hydroquinone overuse can all trigger rebound pigmentation that's darker and more stubborn than the original. This is why so many people spend years trying products that promise results, only to watch their melasma return darker.

The right protocol respects how sensitive melanocytes really are. We start gentle, we layer treatments intentionally, and we prioritize long-term suppression over short-term whitening. Most importantly — we focus on the maintenance phase as much as the active treatment phase, because melasma management is a lifestyle, not a one-time fix.

Common Questions.

Why is melasma so hard to treat?
Melasma is triggered by hormones + sun + genetics. Most treatments only address surface pigment. We treat all layers + prevent return.
Will it come back?
Without maintenance, often yes — within 6-12 months. With our program (treatment + daily SPF + topicals), most clients see lasting results.
Can I use IPL on melasma?
IPL can sometimes worsen melasma. We use safer modalities — cosmelan, gentle microneedling, oral tranexamic acid.
How long until I see results?
Visible improvement in 4-6 weeks. Dramatic results after 3-4 sessions. Continued improvement over 6 months.
Is melasma related to pregnancy?
Often yes — called 'mask of pregnancy.' We avoid treatment during pregnancy, treat post-partum once breastfeeding ends.
